South Georgian Bay CHC

The South Georgian Bay Community Health Centre (SGB CHC) is a non-profit organization providing community focused health promotion, illness prevention and client centred primary health care services to residents who face special barriers to health.

The (SGB CHC) services the residents of Blue Mountains, Clearview Township, Wasaga Beach, Collingwood and Elmvale.  A range of community-based health services are provided by a multi-disciplinary team including physicians, nurse practitioners, nurses, a social worker, dietitian, physiotherapist, health promoter, diabetes educator, mental health and addictions counsellor, a community youth services coordinator, administrative staff, our dedicated volunteers and you.
